This can happen on occasion if the attachment for the Bank Return Notification is being uploaded around the same time that Vantaca searches and pulls the notification from the bank into Vantaca as an action item.
If you see this happen on a Notification action item, the attachment will likely show on a Notification action item the following day, as Vantaca's process for searching and pulling Bank Return Notifications does so with a wide date range.
